** This form is only applicable to Practitioners who are already qualified to Practitioner 1 and who are current members of the Guild.

Please check website course dates to ensure that Assistant places are still available before sending in the form.

Since places are limited and fill up very quickly, sending in an application form does not guarantee a place. Places will be confirmed by the office.

Please note that deposits are NON-REFUNDABLE and NON-TRANSFERABLE. Cheques for the Practitioner Training Courses should be made payable to TTEAM.

*** Tilley Farm is unable to accept Credit Card payments, for Training Courses, at the present time.***
